Thank you very much for taking the time to fill out this form, the information you provide is very important to us and we appreciate you doing it.  

The LTD Program is a serious commitment and is not suitable for all young people, so please help us discover if the potential student is ready to join us as a Level One Participant or continue in level two or three. One of our main purposes for getting a reference from you is to help us in our efforts to treat each participant as an individual with uniqueness, as well as strengths and growth areas.
